The Log of Work-Related Injuries and Illnesses (OSHA 300 Log) is used to classify work-related injuries and illnesses.
The Summary (OSHA Form 300A) is a separate document and shows the totals for the year in each category.
In Oregon, an employer is required to complete OSHA Form 300 (and Form 300A) if they have 11 or more employees at any point during the calendar year. Companies with 10 or fewer employees at any point during the calendar year are generally exempt from keeping OSHA injury and illness records.
Even if there are zero injuries or illnesses, you are still required to complete and retain Form 300 and Form 300A and post 300A on your central posting location.
Employers must keep a log for each establishment or site. If you have more than one work site, you must keep a separate OSHA 300 Log and Summary for each physical location.
You must keep Form 300 and Form 300A for five years.
Yes, you must post ONLY the summary (OSHA Form 300A) by February 1st of the year following the year covered by the form and keep it posted until April 30th of that year. For example, the form for 2024 must be posted from February 1st – April 30th in 2025. This summary should be posted in a common area (such as your central posting location).
Do not post the OSHA 300 Log, as this contains confidential information (employee names, illness/injuries).

- Death
- Loss of consciousness
- Days away from work
- Restricted work activity or job transfer
- Medical treatment beyond first aid
